Employee Benefit Programs
Careers at The Miriam Hospital
The
benefits outlined below are available to employees working standard
hours of forty or more per bi-weekly pay period. Most benefits become
effective on the first of the month following date of hire. Where
applicable, benefits are pro-rated for employees working less than 80
standard hours but more than 40 standard hours per bi-weekly pay period.
Benefit Choices

Health Insurance
Your choice of health insurance includes Lifespan Blue or
cash in lieu of coverage when proof of alternative medical
coverage is provided.

Dental Insurance
Your choice of dental insurance includes Delta USA Plan A,
Delta USA Plan B, or no coverage.

Long-Term Disability (LTD) Insurance
Eligible employees are automatically covered for LTD insurance
equal to 60 percent of pay to a maximum of $6000 per month. If
you are unable to work for more than 180 days, LTD coverage
coordinates with other sources of income and with benefit
payments you receive from other disability programs like TDI,
Workers Compensation, or Social Security to bring your income up
to 60 percent of your base pay.

Employee Basic Life and Accidental Death and Dismemberment
Insurance
Employees may elect coverage equal to one, two, three, four
or five times annual base pay up to a maximum of $500,000, or no
coverage. For those employees who take supplemental coverage,
free will preparation services are available.

Dependent Basic Life Insurance
Coverage for your spouse and dependent children up to the
age of 19 (or age 23 if a full-time student) is available in the
following amounts:
Spouse: $10,000, $25,000, or $50,000
Child: $5,000* or $10,000*
*Children 14 days to 6 months of age will have a benefit equal
to $1,000. At 6 months of age, the benefits will equal the
coverage amount selected.
CHOICE Retirement Plan

The CHOICE Core Account provides an annual contribution
from 2 percent to 6 percent of annual pay based on years of
credited service. You become eligible on the first day of the
month after you complete one year of service and have worked at
least 1,000 hours in the 12-month period starting with your
first day on the job.

The CHOICE Matched Savings Account allows employees to
save pre-tax dollars through automatic payroll deduction. There
is no waiting period. You can choose from 49 Fidelity investment
options. Lifespan matches your contributions at $0.50 on each
dollar of the first 4 percent of your contribution.

The Supplemental Tax-Sheltered Accounts allows employees
to tax shelter savings in TIAA-CREF.
Paid Time Off

Nine Paid Holidays
Employees are eligible for the following paid holidays:

Premium Pay
If you work on the holidays listed above (excluding Yom
Kippur), you will be paid a premium equal to one half of your
hourly rate in addition to your regular pay.

Earned Time
Benefit-eligible employees begin to accrue earned time with
their first paid hour of employment or transfer to
benefit-eligible status. The amount of earned time, which an
employee is eligible to accrue, is dependent on job category,
length of service, and standard hours. Employees are eligible to
begin using their accrued earned time once they have
successfully completed the 90-day introductory period for
scheduled or unscheduled absences according to departmental
policy.

Working Healthy
A variety of health promotion programs are offered to
employees such as smoking cessation, on-site yoga, on-site
weight watchers, free health screenings, on-site fitness
centers, monthly lectures, walking and running clubs, and more.
Since 1998, Lifespan has been an annual recipient of the
“Worksite Health Award,” sponsored by the Greater Providence
Chamber of Commerce and BCBS of RI. Newport Hospital is the
recipient of the “Gold Level Well Workplace” award for 2005-2008
and each other Lifespan affiliate is the recipient of the
“Silver Level Well Workplace” award for 2006-2009 as designated
by the Wellness Council of America.

Employee Fitness Program
The Cardiac Fitness Center is open to employees during
designated hours with no joiner's fee and $5.00 per week with
payroll deduction. The facility offers an indoor track,
free-weights, a weight training circuit, more than 70 pieces of
cardio equipment as well as free classes.

Employee Health Services
Optimal health services are provided for Lifespan
employees, promoting occupational effectiveness through a
multi-disciplinary approach to support health and minimize
risks, and providing medical guidance with an understanding of
Workers' Compensation laws, occupational health safety, and
regulations pertinent to accreditation requirements.

Employee Assistance Program
This free, confidential service isavailable as a benefit to
all Lifespan employees and their family members who need
information, or assessment and referral to counseling or other
resources. In addition, the service includes a resource and
referral program for childcare and elder care.

Lactation Support Program
Lifespan is dedicated to fostering healthy babies and
families, and is proud to offer parents additional support
through our Workplace Lactation Program. The Miriam Hospital has
been recognized by the Department of Health as a Breastfeeding-Friendly
Workplace, winning the 2007 Silver Award.
Child Care, Elder Care & Family Support

Resource and Referral Services
The Employee Assistance Program offers resource and
referral services for child care and elder care in addition to
its confidential service to Lifespan employees and their family
members who need information, or assessment and referral to
counseling or other resources.

Dependent Care Flexible Spending Account
Employees can contribute, on a pre-tax basis, up to a maximum of
$5,000 over the course of the year to be used as reimbursement
toward eligible expenses such as child and elder day care, day
camps, preschool and after-school care.

Dependent Life Insurance
Coverage is available for your spouse and dependent children up
to the age of 19 (or age 23 if a full-time student).

Long-Term Care Insurance
Long-term care is not one service, but many different
services aimed at helping people who have chronic conditions and
are unable to care for themselves. Services may be needed within
the home for day-to-day activities or in a nursing home when
special attention is needed. Long-term care services are not
always covered by major medical plans or are severely limited
within Medicare and Medicaid.

Education Assistance
Full-time employees are eligible for reimbursement of 75
percent of approved tuition costs up to $1,250 per calendar
year. Part-time employees are eligible for reimbursement of 75
percent of tuition costs up to a maximum of $625 per calendar
year.

Relocation Assistance
Individuals who are offered, accept and begin full-time
employment may be eligible for reimbursement towards approved
relocation expenses.

Computer Purchase Discount Program
Employees can purchase computers and related products at
discounted prices.

Financial Planning & Legal Services
With legal services you can help protect your rights in
some of life's most frustrating situations. You can protect
yourself and your financial security if expensive legal services
ever become necessary.

Health Care Flexible Spending Accounts
Employees can contribute up to a maximum of $3,000 (tax
free) over the course of the year to be used as reimbursement
toward eligible expenses such as deductibles, coinsurance, and
co-payments, as well as other health expenses that are not paid
by insurance, such as vision care, eyeglasses and contact
lenses.

Personal Insurance Program
Employees receive a group rate for homeowners, car and boat
insurance and can participate through payroll deduction.

HIV Insurance
HIV insurance provides a lump sum payment should you test
HIV positive while covered by the plan. The benefit provides
24-hour coverage and is payable in full at the time of the
claim. If you elect this coverage, the insurance company will
require a confidential, valid, negative test result for HIV. The
test must be performed by a representative from the insurance
company.

MyHealthAdvocate
This service provides expert help, guidance and support to
make sure employees get the most from their Lifespan health and
pharmacy plan.

Employee Fund
Whether it be a fire that has destroyed your home or a
serious illness that has stricken your child, your fellow
employees want to help. The Employee Fund was created to provide
financial assistance when an unexpected catastrophic event has
left them with nowhere else to turn. Unlike employee loan
programs, money granted through the Employee Fund does not have
to be paid back. The fund is managed by a committee of diverse
employees who determine whether to grant or deny a request based
on established guidelines.

Work/Life Lunch and Learn Sessions
Each Lifespan affiliate offers quarterly programs on topics
such as College Planning, Caring for Elders, Balancing Work and
Family, Advance Directives, How to Talk to Your Children About
Risk Taking, Managing the Holidays, Gardening 101, Adoption and
Foster Care, and more.
