Requests for Medical Records

If you would like copies of your medical record, please call the medical records department (Health Information Services). You may pick up the copies in person with a valid driver's license or other picture ID, or copies will be mailed to you.

We cannot fax or e-mail medical records without written consent from the patient (this must be done in-person at the time of your visit and varies by department).
