Care Management

The care management department helps make plans for your return home or, if needed, to a post-acute care facility. Care managers can answer your questions about discharge planning and can arrange for home health care services, visiting nurse services, transfers to rehabilitation programs, transfers to skilled nursing facilities, nursing home placement, home medical equipment, home hospice care and inpatient hospice care.

Care management staff may meet with you and your family to discuss your wishes and work with your health care team to prepare you and your family for your discharge from the hospital. If you haven’t met with a care manager you can call 793-2070 (internally dial ext. 3-2070) to discuss your needs. We have information on community and state agencies and other support services, and can help determine if you are eligible for assistance.

It is our belief and our policy that you should participate fully in the decisions regarding your care, both while you are here and when you are discharged. Our goal is to ensure that you receive the best care and that your needs and choices are respected by our physicians, nurses, care managers and support staff.

Discharge Policy

To prepare for patients being admitted, we ask that you plan to depart as early as possible in the morning of the day you expect to go home.

You cannot be discharged until your physician completes the necessary medical records. On occasion, we do experience a delay in the discharge process; however, we request that a family member or friend arrive in the morning.

Lifespan Pharmacy

Lifespan Pharmacy can fill and deliver your discharge medications to your bedside at no extra cost to you. If you prefer, you (or a family member or caregiver) may also pick up your medications at Lifespan Pharmacy, which is located in the main lobby at The Miriam Hospital. This is an optional program that is offered for your convenience. Please note that you have the right to select any pharmacy you wish to fill your prescriptions. If you need more information before making a decision, feel free to contact the pharmacy staff at 401-793-5500 (The Miriam Hospital) or 401-444-4909 (Rhode Island Hospital). To choose Lifespan Pharmacy bedside delivery service for your discharge medications, fill out the Lifespan Pharmacy information
sheet to opt in for bedside service. Your prescriptions will be sent to Lifespan Pharmacy to be filled. Your medications will be delivered to you before you leave.

How long will I have to wait for my prescription?
We begin processing your prescriptions as soon as you request that they be filled at Lifespan Pharmacy. Your prescription will be delivered to you before you leave.
 

Once I have my prescriptions filled, how do I get my refills?
We know it may not be convenient for you to come back here for your refills. Lifespan Pharmacy makes it easy to get your refills any time, day or night, and offers free home delivery.

Will my discharge prescriptions go on my hospital bill or are they separate?
Your discharge prescriptions filled at Lifespan Pharmacy are separate from your hospital bill. We will bill your prescription insurance first and you will be responsible for any co-pays upon receipt of the medications.

Follow-Up Care

If your physician recommends follow-up care after you are discharged, it is your right to choose the provider of these services. You may decide to continue to receive care through other Lifespan affiliates, including:

Outpatient Nutrition: 

Counseling with a registered dietitian is available for all diagnoses. Persons interested in nutrition counseling for diabetes should call Terry Jerominek at 401-793-8796. For general outpatient nutrition counseling, please call 401-793-2621.

We understand that you may be familiar with another provider or have a preferred provider based on your insurance plan. Our care management staff will give you a list of licensed and/or certified health care providers in the state in which you reside and assist with your selection. Because we are unfamiliar with the practices of the providers on the resource list, we cannot make any recommendations.
